Drug addiction HELP!!!!?

ok for the past 2 years i have had a lot of problems with every drug you can think of! shrroms,x,coke,heroin,marijuana,perscrip… pills,lsd,alcohol and even inhalents. and it just started out as trying them. but i went to rehab 3 times and the withdrawls are just too painfull. i feel like im never going to quit. im high on something everyday all day . i barely ever sleep from all the drugs and now i dont know wut to do. im really scared im going to die from this. can someone help me out?
